Coaches

Coaches must complete the KidSafe form, SAGE form and Concussion Awareness Training in order to coach youth soccer.

 

KidSafe: Every coach must download the KidSafe form and complete it.

 

SAGE Form: Every coach must download the SAGE form and complete it.

 

Concussion Awareness Training: The State of New Jersey requires coaches to have head injury safety training to help in the recognition of concussions.  Therefore all coaches will be required to have taken the on line Concussion Awareness Training and be able to present their certificate they receive from taking the course.

 

Parents

There are two forms that parents need to download, complete and give back to their player’s coach. The forms are located to the left.

 

SAGE Form:  Each family must fill in all the requested personal information and sign. Note: One form per player is needed and both the player and parents/guardians must sign.

 

US Youth Soccer Medical Release Form: Please Fill out all personal information. This form does not need to be notarized.
